Please complete a Travel Risk Assessment Form from reception at least 6 weeks prior to travel. 

This will need to include which countries and areas within countries that you are visiting to determine which vaccinations are required.

It is important to return the Travel Risk Assessment Form back to the surgery, at least 6 weeks before you travel so that the practice nurse can assess what risks you may face and arrange an appointment for advice and any vaccinations you may need.

Some vaccines are free to NHS patients. Others may need to be ordered, and these incur charges.

Malaria prevention medication may be recommended for some countries, also incurring a charge.
